Registered Nurse Case Manager

About the role

We are seeking a compassionate Registered Nurse to serve as a Case Manager, delivering in‑home care to former Nuclear Weapons Workers living with chronic or terminal illnesses. The role combines direct patient care with coordination of multidisciplinary services to ensure safe, effective, and personalized support.

Key responsibilities

• Conduct weekly in‑home nursing visits, performing comprehensive physical assessments.
• Develop, implement, and monitor individualized care and medication plans.
• Assess and maintain home safety, addressing environmental hazards.
• Supervise and coordinate caregivers, ensuring adherence to care protocols.
• Collaborate with physicians, therapists, and social workers to align treatment goals.
• Document clinical findings, update care records, and contribute to performance‑improvement initiatives.
• Participate in staffing activities, including interviews and training sessions.

Required profile

• Graduate of a state‑approved nursing program with a current RN license.
• Minimum two years of nursing experience, including at least one year in home‑care settings.
• CPR certification.
• Preferred BSN degree and prior supervisory experience.
