Registered Nurse - RN Case Manager

About the role

We are seeking a compassionate Registered Nurse to serve as an RN Case Manager, delivering in‑home care to former Nuclear Weapons Workers living with chronic or terminal illnesses caused by toxic exposure. The role combines direct patient care with coordination of multidisciplinary services to ensure safe, high‑quality support for each client.

Key responsibilities

• Conduct weekly in‑home nursing visits and perform comprehensive physical assessments.
• Develop, implement, and monitor individualized medication plans.
• Assess and maintain home safety, addressing environmental hazards.
• Supervise and support caregivers, providing guidance and training.
• Coordinate care with physicians, therapists, and other team members.
• Document clinical findings and update care plans in accordance with agency policies.
• Participate in staff training, performance improvement initiatives, and occasional supervisory visits.

Required profile

• Graduate of a state‑approved nursing program with a current RN license.
• Minimum two years of nursing experience, including at least one year in home‑care settings.
• CPR certification is mandatory.
• Preferred BSN degree and prior supervisory experience.
