Community Health Nurse – Preventive Care

About the role

The Community Health Nurse will deliver preventive, promotive and basic curative services to residents of Accra, Tema and surrounding communities. This role focuses on health education, disease prevention, immunisation, and maternal‑child health support while ensuring accurate record‑keeping and quality patient care.

Key responsibilities

• Conduct community outreach and health education sessions.
• Provide immunisation services and basic curative care.
• Support maternal and child health programmes, including antenatal counselling.
• Identify health risks, refer patients to appropriate facilities, and follow up on referrals.
• Maintain accurate patient records and report health data to the facility.

Required profile

• Diploma in Community Health Nursing.
• Minimum of 2 years relevant nursing experience.
• Residence within or near Achimota, Lapaz, Taifa, Tantra Hill, Ofankor Barrier, Pokuase, Amasaman, Dome or nearby areas.

Required skills

• Proficiency in basic nursing procedures and patient care.
• Ability to conduct health education and community outreach.
• Knowledge of immunisation protocols and maternal‑child health guidelines.
